The bottom line is, he hasn't changed because he doesn't have to and apparently doesn't want to. This will not go away by itself.

You could try to dig into the psyche and diagnose him, but the bottom line, he needs external motivations (maybe a gentle nudge, maybe a swift kick in the *** - I suspect something closer to the latter).

Have you asked what he plans to do when his parents are gone? Because I suspect he's given zero thought to the notion, and maybe some nudging to come to the conclusions about how unemployable he is (and is not becoming more so by any means) would leave him completely unprepared in the worst way possible.
