Found this in the Android Marketplace and thought I'd share. It sounds like it's available for iPhone and possibly WinMo as well.

Full featured and customizable for tracking Medications/dosage (i/e insulin), Blood Glucose, HeA1c, Weight, Blood Pressure, Carbs, Calories, Exercises, etc...

Reports are fully customizable, filterable, and, exportable to .csv, .xml, or .html, and can be emailed!!!
