As pictured, can't be done that I can think of. Too many angles, requiring an odd number of connecting piercings, and no way in hell to put that jewelry together that seamlessly.

Plus, imagine snagging that big a rock on something...

Actually, if you assume there is a seam where the little metal ball is and then a 3 piece connection under the skin, its actually feasible.

I think "cut the skin open and then let it heal over the ring" is actually the easiest solution to this problem. Everything else seems too far-fetched and impractical to me.

Doing it in the first place is far-fetched and impractical. That's even more utterly stupid than most body piercing.

The ball isn't at a place that could feasibly be used to secure the piece. Look at the joints on the ring...how are you going to insert the jewelry into the piercing? And then...do you really want to try to assemble the 3 way joint blind and in the piercing itself? With only skin to work with and no flesh?
